Information you give us

When you use the contact form or the new patient booking form, we ask for your name, email address and phone number, and on the booking form your date of birth, the specialty you are interested in and the general area of your pain. The forms tell you not to include medical details, and we ask that you keep them for your visit.

We use this information to respond to your request, to schedule and confirm your appointment, and to verify your insurance benefits before your first visit.

Where that information goes

• Your name, email, phone number, the form you used, the page you sent it from, and the campaign details described below are stored in a database we control, hosted by Cloudflare.
• The full submission, including date of birth, specialty and pain area, is emailed once to our front desk so we can call you back. Those details are not stored by the website.
• Your name, email and phone number may also be recorded in WhatConverts, the lead-tracking service we use to keep track of calls and web requests so that nobody is missed.

How we contact you

We use the name, email address and phone number you give us to respond to your request, schedule your appointment and verify your insurance benefits. Submitting a form does not sign you up for marketing emails or text messages.

Calling us

Phone numbers shown on this website may be tracking numbers that forward to our office. Calls to those numbers, and the number you call from, may be recorded by WhatConverts so we can measure which advertising brings people to us. If a call is recorded, you will hear a notice at the start of the call.

Cookies and analytics

We use Google Tag Manager to load Google Analytics 4 and Google Ads conversion tracking. These tools set cookies and collect information such as the pages you visit, how you reached the site, your approximate location and your device and browser. Google Ads uses this to measure whether an ad led to a call or a form request. We may also use Microsoft Clarity to see how visitors move through pages; Clarity records interactions such as clicks and scrolling, not the text you type into forms.

If you arrived from an advertisement, the campaign identifiers in the link (such as utm parameters and the Google click identifier) are kept for your visit and attached to any request you send, so we know which campaign led to it.

You can block or delete cookies in your browser settings. You can also opt out of Google Analytics with Google's browser add-on and manage ad personalization in your Google account.

Spam protection

The forms use Cloudflare Turnstile to tell people from automated bots. Turnstile may process your IP address and browser characteristics to do this.

How long we keep information

Form submissions are kept for as long as we need them to handle your request and to keep our records, and consent records are kept for at least five years. Analytics data is retained according to Google's and Microsoft's retention settings for our accounts.

Who we share information with

We share information only with the service providers named above, who process it on our behalf: Cloudflare (hosting, database and spam protection), Amazon Web Services (email delivery, under a business associate agreement), WhatConverts (call and lead tracking), Google (analytics and advertising measurement) and Microsoft (Clarity, if enabled). We do not sell your information, and we do not share it with other businesses for their own marketing.

Your choices and rights

You can ask us what information we hold about you from this website, ask us to correct it, or ask us to delete it, subject to any records we must keep. Contact us by phone or through the contact form. Under New Jersey law you may also have the right to opt out of the sale of personal data or targeted advertising; we do not sell personal data.

Children

This website is not directed at children under 13, and we do not knowingly collect information from them online. Appointments for minors are arranged by a parent or guardian.

Changes to this policy

If we change how the website handles your information, we will update this page and the date at the top.
